2x40 (no mercy rule yet)
If tied then 2x10 (no golden goal)
If tied then 2x5 (golden goal)
If tied then KFTM (PKs). Now here's where it gets tricky:
If a player is cautioned DURING KFTM, he must wait for an additional round before he can shoot again.
In NFHS there is no reduce to equate principle. Some states record the missing player as an automatic miss. Happy...concur? Or what does LHSAA do?

Here is the exact wording: (TL;DR notes at bottom)
MISCONDUCT PROCEDURES: 1) Cautioned Players a. One of the five players listed to take a kick is cautioned after he takes the kick. That player is not eligible to kick again until one complete set of five kicks has been completed beyond the set of kicks in which the caution was issued. (1) Player X is one of the first 5 kickers and is cautioned after taking his kick. Player X is not eligible to kick again until the set beginning with the 11th kicker. (2) Player Y is one of the second set of 5 kickers and is cautioned after taking his kick. Player Y is not eligible to kick again until the set beginning with the 16th kick. b. One of the 5 players listed to take a kick is cautioned before he takes the kick. That player is not eligible to kick until the next set of 5 kicks. Another eligible player not listed in that set of kicks must take the cautioned players original turn. c. When a goalkeeper is issued a caution during any set of 5 kicks, he cannot re-enter until the next opponent completes his kick. A substitution is permitted for the cautioned goalkeeper.
2) Disqualified Players a. Any player, including the goalkeeper, who is disqualified may not participate any further. If his kick is not completed, an eligible substitute is permitted.

Also, of extreme importance:
Tie Game Procedure 3b - Each coach will select any 5 players, including the goalkeeper, on or off the field (except those who may have been disqualified) to take the kicks.

Again, of importance/relevance:
Rule 14-4 Once the kicker starts his aproach toward the ball, he may not interrupt his movement. Failure to kick the ball as specified shall result in a rekick**

Summary
-If a card is issued during KFTM it affects the order, depending on when it was issued.
-No reduce to equate principal. Not counted as a miss either. In fact it clearly states a substitute is permitted.
-Goalkeeper caution, he must skip the next kicker. Substitute permitted.
-Any rostered player, on or off at expiration, can kick. - The stutter-step is NOT permitted and MAY result in a caution

There is nothing in the rules that says the referee has to be notified of the order of kicks. So, unless player is en route to take the kick when he is cautioned (I am sure no young lady would fall foul of this rule), or unless he is player 5 in the rotation, then the substitute could be last to kick.

The order is not covered, therefore, you can make your decision under 5.3.2g and be correct.
