At our match tonight, the referees allowed our opponents to kick the ball or throw the ball after a foul or out of bounds was called.
The last time I checked, this should have been a yellow card?

By the letter of the law: yes, however, there are many factors to consider. Ball goes out of bounds and player kicks it further, but there's a ball boy right there throwing your team another ball - play was never delayed so no card warranted
Foul near the 18 yard line in a close game which typically warrants a ceremonial free kick and for the teams to wait on the referee's whistle anyways - I'm more worried about player extracurricular activity in that area and trying to sort out the wall and avoid confrontation after the foul.
Your team is winning and wins a free kick and the opponent kicks the ball further - they're delaying and losing their own time to try and equalize. Not going to penalize them for wasting their own time.
Player kicks the ball after a foul and it goes near another player on your team who kicks the ball back to where the foul occurred - such is life, not going to delay the game further and warrant a card in that scenario.
Now if little Timmy boots the mess out of the ball and it sails 30 yards away from the touchline or where the foul occurred, I'm probably going to have a card with little Timmy's name on it at that point.
It all depends on the situation and nature of the game and does the game and player management at that point warrant a card.

How about teams consistently standing in front of a ball that has been whistled dead for a free kick. Played a game last year in which the team we played would stand on top of the ball every time the whistle was blown for a foul, delaying a quick restart. It was intentional and the referee refused to warn them. There was 0 attempt to give 10 yards. They also were moving around in front of the ball, kicking out feet, jumping, moving around the ball. What's your thoughts on that?

Personally, I give one warning to tell the players to back off the ball and get out of there. Second time I see it happens, the yellow comes out and they typically quit doing silly things like that afterwards.

If you are affiliated with the team in an official capacity, have a conversation with the assignor about what occurred, date/time of match, and officials name of you know. This gives the assignor factual info they can use to address with officials. If you are not officially affiliated (but just a parent or fan), ask the coach or school admin to address with assignor. If you have video evidence, provide that as well.

These were of the "let's kick it 40 yards down the field" variety over and over again all night. I brought up the yellow card rule, and the response was "No... no yellow." After becoming a referee, it has opened my eyes even more to the referees who don't pay attention to the rules. They also didn't know that a throw-in isn't retaken if it doesn't enter the field. Nor did they seem to know that patting the ball down to the feet by using their hands was illegal. Advantage was also given until we were about to score then they promptly pulled our scoring opportunity back about 20 yards for a free kick a couple of times... Oh... and after watching the film today, they didn't seem to know what should be an offside call either.
