If a fan asks a referee his or her name does the ref have to give their name?
How does anyone know that the refs are legitimately who they say they are? Do the referees have a picture ID?
Is there a governing body over these referees, and if so how do you contact them?

No. If you have an issue with a ref, convince your coach or AD to address the issue with the referee organization.

For USSF matches, the governing body is U S Soccer, administered by the various state soccer associations, and in louisiana we have the state referee committee which oversees referee stuff. There are local assignors who assign only registered referees to matches.
For high school, referees belong to the LHSSOA, which falls under LHSAA, which follows NFHS. High school associations are broken down geographically across the state and each has an assignor.
In both cars, assignors know the referees.
If you have an issue with an officials, have it addressed via the coach, team manager, or school or club official. They will contact the assignor who will know what referee was on said game.

I All you need to provide to the assignor is the date, time, location, teams, and a valid complaint. We can usually figure out the rest.
